Back to School Packet Forms

All Medical Forms and Back to School Form must be in before August 15th.

Forms can be returned by - email to records@craigschool.org, dropped off at the Blue House- Administrative Offices, Mon-Fri 8:00-3:00 during the summer, or by mail

If you drop off or mail your forms we recommend making a copy for your records.

If you need a hard copy of the packet please let us know as soon as possible so we can make arrangements for pick up, or mailing. Send your request for a hard copy to records@craigschool.org
